Each quarter, Victorian community health providers must report data on their funded services to us.

They must follow our guidelines on what they report and how frequently as outlined in the submission guidelines below.

Service providers send their data submissions via the Community Health Minimum Data Set (CHMDS) to us on a quarterly basis.

We then compile quarterly performance reports from the data sent via the CHMDS, and then post the reports to the Funded Agency ChannelExternal Link .

The quarterly data submissions due dates are:

  • Quarter 1 - July to September: due 1-15 October
  • Quarter 2 - October to December: due 1-15 January
  • Quarter 3 - January to March: due 1-15 April
  • Quarter 4 - April to June: due 1-15 July

The Community Health Minimum Data Set submission guidelines and transmission protocol have been updated for 2023-24. These and other documents can be found below under Downloads.

Data specifications

Please note the number of client records in your xml files that can be uploaded has increased from 1000 to 5000 records. This will reduce the number of files agencies need to submit within each quarter.

When submitting multiple files, ensure that the Export File Portion number is increased by one digit for each file in order for the files to be appended to each other.

Using the same export file portion number when submitting multiple files will cause the files to overwrite each other.
