Women are encouraged to be involved in decisions about their care, and to improve the communication between the various care providers involved in maternity care.

The Victorian Maternity Record (VMR) provides pregnant women with a standardised maternity record of their pregnancy care and progress. The VMR:

  • reduces risks by providing health services with access to a complete pregnancy record
  • improves the quality and consistency of information given to women
  • enhances continuity of care
  • improves communication between service providers
  • provides continuity of documentation between clinicians and sites.

Public maternity services should tell all pregnant women about the VMR and encourage them to use this resource.

Ordering Victorian Maternity Records

The Victorian Maternity Record can be ordered online by filling out the details below and pressing "Submit" at the bottom of the page.

We do not ship outside of Victoria.

  • The minimum order is 20.
  • We do not ship outside Victoria or process orders as urgent.
  • Keep a copy of your order number to assist with any queries.
  • For any queries about your order, you can email maternity@dhhs.vic.gov.au.
  • An external distribution organisation handles the online ordering process.
  • The Department of Health (the department) is committed to protecting your privacy. The department collects and handles your personal information for the purposes of responding to your request for a hard copy of the Victorian Maternity Record (VMR), which is published by the department.

    Any personal information collected by the department will be used and handled in accordance with the Privacy and Data Protection Act 2014 (Vic) and the department’s privacy policy. The department will disclose your personal information and your order details to an external warehouse and distributor contracted by the department to print and distribute VMRs.

    If you do not provide us with the information requested in this form, we will not be able to respond to your request for a VMR.

* Please note that the minimum number of VMR kits that can be requested in a single order is 20. Ordering 50 VMR kits and above, in increments of 50 (ie 50, 100, 150, 200) is preferred. The minimum number of VMR forms that can be requested in a single order is 100. Ordering VMR forms in increments of 100 (ie 100, 200, 300) is preferred.
