The Hume region health planning guide provides a consistent approach to health planning, featuring a health planning cycle with key steps in the planning process. The guide identifies seven critical success factors in successful health planning. The guide also includes health planning approaches in Hume health region.

About Hume health region

Hume health region is located in the northeast of Victoria and has four sub-regional planning catchments known as primary care partnerships (PCP):

  • Central Hume
  • Goulburn Valley
  • Lower Hume
  • Upper Hume.

Each of the PCP catchments span a number of local government areas.
