The Department of Health and Human Services has developed training and e-learning packages to help Victorian health services and their staff with the principles and processes of clinical risk management.

Fundamentals of root cause analysis investigations

An introduction to root cause analysis methodology comprising an overview of incident investigation processes, developing a problem statement, identifying critical events, understanding cause and effect principles, writing causal statements and forming recommendations.

Who should attend

Health service representatives with minimal or no previous experience in or knowledge of root cause analysis investigations.

Prerequisites

Participants must understand the seven steps in effective incident management and how the ISR guides the level of investigation. See the Victorian health incident management policy guide.

Participants must also complete the following clinical risk management e-learning packages:

  • VHIMS e-learning
  • Open disclosure e-learning
  • In-depth case review e-learning.

Leading a root cause analysis investigation

An online session for health service representatives responsible for leading root cause analysis investigations including assembling the RCA team, coordinating the investigation, gathering evidence, facilitating meetings and developing a report.

Who should attend

Health service representatives with prior experience in and knowledge of root cause analysis investigations who are or will be responsible for facilitating an investigation.

Prerequisites

Participants must have previously attended introductory root cause analysis training or had previous experience as a RCA team member.  

Online sessions will be held using a WebEx virtual meeting. You will need access to a PC with audio/webcam and a reliable internet connection.

Root cause analysis refresher

An interactive online session providing a refresher on the fundamentals of root cause analysis investigations with the opportunity to ask specific questions, examine scenarios and discuss particular concepts in more detail. The content covered during this session is flexible and participants are encouraged to submit requests prior to the session.

Who should attend

Health service representatives who would like a general refresher on RCA concepts or with specific questions they would like answered.

Prerequisites

Online sessions will be held using a WebEx virtual meeting. You will need access to a PC with audio/webcam and a reliable internet connection.
