Safer Care Victoria manages and supports a number of consultative councils and ministerial advisory committees.
Safer Care Victoria also manages the process to appoint new members to these councils and committees, and will advertise calls for nominations when there are vacancies on these committees.
The Minister for Health decides the final membership.
The 2018-2019 term applications for council membership for the Victorian Consultative Council on Anaesthetic Mortality and Morbidity and Victorian Surgical Consultative Council are now open.
Documents required for VCCAMM and VSCC
Applicants will be required to complete and submit the following documentation:
- Current Curriculum Vitae
- Application form for appointment to public entity boards
- Declaration of Private Interests form
- National Police Checking Services (NPCS) application consent form
- National Police Checking Service proof of identity (special provision) for Aboriginal and Torres Strait Islander People
- Safety screening and Supplementary consent form
*Note: Only clear, colour copies of proof of identity documents will be accepted. The documentation you provide must include evidence of your full name and date of birth and a photograph of you. The commencement and primary documents provided must include a photograph of you. If your commencement and primary documents so not include a photograph you must submit a certified passport-style photograph. If poor quality proof of identity documents are received you may be asked to resubmit. To complete the 'National Police Checking Services (NPCS) application consent form download the file to your desktop first.
If you are shortlisted, you will be required to provide copies of police checks from those countries in which you have lived for more than 12 months in the past 10 years (as indicated in the SI form). Further information about this can be found under 'character and police certificate requirements’ at the Department of Immigration and Border Protection website.

The VCCAMM provides advice to the Minister for Health, Safer Care Victoria and the Department of Health and Human Services on avoidable causes of morbidity and mortality related to anaesthesia.
The VSCC Council monitors, reviews and analyses reports of surgery-related mortality and morbidity. It also investigates any matter referred by the Minister for Health, Safer Care Victoria and the Department of Health and Human Services.
