Over the past 50 years, Victoria has developed a highly integrated system for monitoring and reporting on maternal, perinatal and paediatric deaths, and for advising the government on strategies to improve the safety and quality of obstetric and paediatric care.

The Consultative Council on Obstetric and Paediatric Mortality and Morbidity (CCOPMM) reviews all cases of maternal, perinatal and paediatric mortality and morbidity, and advises the Minister and the Department of Health & Human Services on strategies to improve clinical performance and avoid preventable deaths.
