The department has established three online collaborative groups to inform the roll out of HealthLinks: Chronic Care. If you represent a health service that is participating in HealthLinks: Chronic Care, you can join one of these groups through the department's Extranet.

Clinical Collaborative

The purpose of the HealthLinks: Chronic Care Clinical Collaborative is to provide clinical oversight and leadership for HealthLinks: Chronic Care. Specifically, the collaborative will:

  • support a cooperative approach to sharing HealthLinks Chronic Care tools, processes and learnings
  • provide clinical advice on the design of interventions and commissioning models
  • identify best practice and protocols
  • facilitate changing models of care
  • improve links between hospital program areas (emergency, acute, subacute, substitution and diversion services) to improve care pathways for people with chronic and complex conditions.

The group consists of senior managers from participating health services.

Operational Community of Practice

The purpose of the HealthLinks: Chronic Care Community of Practice (Operational) is to provide a forum to share operational intelligence and practical strategies to successfully implement HealthLinks: Chronic Care.

Specifically, the community of practice will:

  • support a cooperative approach to sharing HealthLinks: Chronic Care tools, process and learnings
  • share operational advice
  • share best practice, protocols and processes
  • support the implementation of changing models of care
  • improve links between hospital program areas (emergency, acute, subacute, substitution and diversion services) to improve care pathways for people with chronic and complex conditions
  • provide relevant feedback to the HealthLinks: Chronic Care Clinical Collaborative
  • If operational themes/issues occur, make relevant recommendations about Healthlinks: Chronic Care implementation to the Clinical Collaborative and to the department.

The group consists of staff that are involved in operationalising HealthLinks: Chronic Care.

Data and Analysis Community of Practice

The purpose of the HealthLinks: Chronic Care Community of Practice - Data and Analysis (CoPDA) is to provide an opportunity to collaboratively develop and share analytics, tools, routines and strategies. Specifically, the Community of Practice aims to:

  • enable participating health services to replicate the department's HealthLinks Chronic Care eligibility and enrolment master lists
  • identify processes and structures that facilitate the early identification of high risk patients
  • ensure consistency of data extraction for the department's HealthLinks Chronic Care evaluation
  • use the (incidental) findings of department's and the health service-level data analyses to identify and propose potential improvements to the HealthLinks Chronic Care approach.

The group consists of  representatives from health services who have responsibility for data analytics within their organisation.

To join one of the above groups, we recommend that you  use a modern browser, e.g.  Chrome or Internet Explorer 11. You may experience issues when using an older browser, such as Internet Explorer 8.

  1. You need to register first at the SectorConnect login page - https://www2.health.vic.gov.au/extranet/login.
  2. Upon registration, you will receive an email notification to verify your email address.
  3. Upon verification, log in to SectorConnect (Extranet) at https://www2.health.vic.gov.au/extranet/login.
  4. To apply for membership of the list, visit one of the links below, scroll to the bottom of the group page and fill in the 'Request to join' box.

If your membership is approved, you will receive access within 24 hours.
