Overview of service coordination tool templates

The Department of Health & Human Services Service Coordination Tool Templates (SCTT) facilitate and support service coordination among health and human services providers. They provide information and data standards, so service providers and the department can electronically share standardised information.

Service providers can use the SCTT to collect and record an initial contact with a person, do an initial needs identification (INI), refer the person, and coordinate the person’s care across services and over time. In this way, the SCTT improves screening and assessment, information collection and sharing, referrals and feedback.

As an example, a service provider can use the following core referral templates to send a referral:

  • Referral cover sheet and acknowledgement
  • Consumer information
  • Summary and referral information.

The SCTT user guide includes information about using the framework and templates to coordinate services for Aboriginal and Torres Strait Islander people.

SCTT 2012

SCTT 2012 is the current version of the service coordination tools and templates. Key differences between SCTT 2012 and the previous version (SCTT 2009) include:

  • a new accommodation and safety template developed by housing and homelessness, family violence and aged care programs
  • a new drug, alcohol and tobacco template developed by alcohol and other drugs program
  • a new single page screener for health and social needs (including service provider and consumer administered versions)
  • amendments that focus on people with a disability, carers, and Aboriginal and Torres Strait Islander people.

SCTT 2012 code sets

The department developed the SCTT 2012 code sets for service providers who are using a paper based version of SCTT 2012.

SCTT 2012 user guide

The SCTT 2012 user guide helps service providers use the templates in SCTT 2012. The user guide is available as a download but is also available in hard copy from your local primary care partnership.

 It provides:

  • an introduction to the templates
  • points for service providers to note in completing the templates
  • support for completing SCTTs for Aboriginal and Torres Strait Islander people.

Training

The SCTT online module is an e-learning tool to support the use of the SCTT. The department encourages service providers to include this module in their staff orientation. Experienced SCTT users may use this e-learning tool to refresh their knowledge, or to keep updated with the changes in SCTT 2012.
