About the grants

The Health Condition Support Grants program provides grants that assist Victorian peer support groups to meet some of the costs associated with providing social, emotional and practical support to members living with a specific health condition.

Grants of up to $5,000 per annum are available for a two-year period to peer support groups focusing on a specific health condition, that provide mutual support and education to members and increase community awareness of the health condition.

Applications for the 2021-23 funding round will open on Monday 3 May 2021 and close on Friday 16 July 2021.

Support groups with a Victorian statewide, regional or local focus may apply. To be eligible, peer support groups must be:

  • focused on a specific health condition that is considered to be a long-term condition with persistent effects
  • operated and administered separately from a larger organisation, for example, have a separate committee of management and separate Australian Business Number (ABN)
  • a group with a specific ongoing membership, not just an activity or education group run by a health or disability agency
  • managed by their members
  • non-profit making.

Step 2: Complete your application via SmartyGrants

The Department of Health is using SmartyGrants to administer the Health Condition Support Grants program. SmartyGrants is an online grant management system that provides easy-to-use online application forms, as well as report management for successful applicants.
